CONTINUING YOUR CARE
If you are currently receiving treatment in our clinic, we encourage you to arrange follow-up care as soon as possible to avoid any interruption in your care. We recommend contacting your primary care physician or your insurance company for assistance in locating a provider who can continue your treatment.

If you choose to continue your orthopaedic care with Dr. Richard Barber or have selected another healthcare provider for your wound care or orthopaedic care, please contact our office to request an Authorization for Release of Medical Records. Complete, sign, and return the authorization form so that we may forward your medical records to the provider of your choice.
Please note that your medical records are confidential and may only be released to another healthcare provider with your written authorization.
